I thought this subject could wait, but it is becoming pressing early than I expected.
As part of our project (and that of similar non-profits - most of which are not open data but nevertheless great organisations), we want to voluntarily encourage cafés, bars and restaurants to offer free tap water bottle refill to anyone off the street. Refill has had significant success in the UK and surprising the feedback is that the impact of increased customer traffic far outweighs any issue of cannibalization.
If it is not already the case, I would like to work with you with on developing a tagging standard for this case which maybe we can vet in a wider OSM audience. Maybe "amenity = cafe & free_water = yes"
It would be important to develop at the same time a distinct tag for another cause, which we support but will not be targeting is restaurants which offer free tap water for paying customers. Maybe "amenity = restuarant & free_carafe = yes"

I browsed a bit through taginfo and there doesn't seem to be a tag already in use, except it seems as if in Hungary free_refill=yes is mistakenly used that way (free_refill=water would be a candidate though).
In any case given that this shouldn't just work for central Europe, maybe this is actually a topic that could be raised on the tagging mailing list. If you don't want to sign up to another list I can ask there.

I'm also interested in this tag because I'm preparing a scavenger hunt for students which is related to the climate debate.
Coming from application and database and design, I'd be cautious with "=yes" tags, because of several reasons I tried to explain here [1]. I would prefer something like free_refill=water (as Simon's proposal) over free_refill=yes whenever possible.

Addendum and sorry: I just read the OSM wiki which points to the double meaning of "free refill" namely as "free refilling a purchased drink" [1][2][3] - versus "offering free tap water bottle refill to anyone off the street"... Now switching to Tagging list... :Stefan
